Welcome to our appointments page! We believe in the power of personalized nutrition to transform lives. Below, you’ll find descriptions of the different types of appointments we offer, including initial assessments, follow-up sessions, and specialized consultations.

Initial Appointment:

• Duration: 45 to 60 minutes.

• Please bring or email any recent lab results and a three-day food record.

• Your nutritional status will be evaluated.

• Your medical and diet history will be reviewed.

• An individualized strategic nutrition plan will be created based on this information to help you reach your overall health goals.

     Please fill out this Nutrition History Form and email it to [email protected] before your first visit.

Follow-up Visits:

• Duration: 30 to 60 minutes.

• We will evaluate your progress in meeting your goals.

• Your food diary and recent labwork will be assessed.

• Strategies will be discussed to support you in achieving your goals.

Cancellation Policy:

• If you need to cancel, please do so at least 48 hours before the scheduled appointment.

• Otherwise, a $200.00 fee will be charged.

• Please be considerate of our time.

Visit Payments (Insurance and Self-Pay):

Insurance:

• Accepted insurance plans include Oxford Health Plans, The Empire Plan (NYSHIP), Empire Blue Cross Blue Shield, and Medicare (for diabetes and kidney disease with a physician referral).

• Contact your insurance company before your first appointment to ensure coverage for nutrition services related to your specific diagnosis.

• If you fail to check with your insurance provider beforehand, you will be responsible for payment at the time of the visit.

• Bring any necessary referral documents to the first visit.

• You are responsible for any visits denied by your insurance carrier.

Self-pay:

• Initial visit (60 minutes): $200.00

• Follow-up visit (30 minutes): $150.00

• Follow-up visit (60 minutes): $200.00
